Abstract and Introduction

Abstract

Study Objectives: To determine the effects of grapefruit juice on the pharmacokinetics of oral digoxin, a P-glycoprotein substrate not metabolized by cytochrome P450 3A4, in healthy volunteers, and to assess whether polymorphic multidrug-resistance-1 (MDR1) expression contributes to interindividual variability in digoxin disposition.
Design: Prospective, open-label, unblinded, crossover study.
Setting: University research center.
Subjects: Seven healthy adult volunteers (four men, three women).
Intervention: Each subject received a single oral dose of digoxin 1.0 mg with water or grapefruit juice with at least a 2-week washout between treatments. During the grapefruit juice phase, juice was administered 3 times/day for 5 days before digoxin administration to maximize any effect on P-glycoprotein.
Measurements and Main Results: Digoxin pharmacokinetics in the presence and absence of grapefruit juice were compared. The MDR1 exon 26 C3435T genotype was determined by real-time polymerase chain reaction. Compared with water, grapefruit juice significantly reduced the digoxin absorption rate constant (3.0 ± 2.4 to 1.2 ± 1.0 hr-1, p<0.05) and increased absorption lag time (0.32 ± 0.12 to 0.53 ± 0.34 hr, p<0.05). Grapefruit juice did not affect digoxin maximum concentration (Cmax), area under the curve (AUC), elimination half-life, or renal clearance. The effect of grapefruit juice on digoxin Cmax (-45% to +41%) and AUC0-4 (-29% to +25%) varied substantially among subjects and was inversely correlated with the values during the water phase. Trends toward higher digoxin Cmax, AUC, and absorption rate constant during the water phase were found in CC homozygotes compared with subjects carrying a T allele.
Conclusion: Inhibition of intestinal P-glycoprotein does not appear to play an important role in drug interactions involving grapefruit juice. Interindividual variability in response to grapefruit juice may be related to the balance of intestinal drug uptake and efflux transport.Introduction

P-glycoprotein (P-gp) is a membrane-bound protein encoded by the human multidrug-resistance-1 (MDR1) gene that acts as an efflux pump to transport a variety of compounds out of mammalian cells.[1, 2] It is highly expressed on the apical (luminal) surface of organs that have excretory functions, such as the bile canalicular membrane of hepatocytes and renal proximal tubules.[2, 3] P-glycoprotein is significantly expressed on the luminal surface of tissues that serve as barriers, such as the brush border of the small intestine and capillary endothelial cells of the blood-brain barrier.[1-5] Intestinal P-gp acts to limit the bioavailability of substrate drugs by pumping them from the enterocyte back into the gut lumen.[6-8] Accordingly, the importance of alterations in P-gp function as a mechanism for drug interactions is widely recognized.[8-12]

Over the past decade many studies documented an interaction between grapefruit juice and drugs that are substrates for cytochrome P450 (CYP) 3A4.[13-18] These investigations showed that mechanism-based inhibition of intestinal CYP3A4 by grapefruit juice increases oral bioavailability of CYP3A4 substrate drugs.[15-19] In addition, evidence suggests that grapefruit juice may affect drug absorption by inhibiting intestinal P-gp.[16] However, conflicting results from other studies indicated that the juice induces, inhibits, or has no effect on P-gp activity.[16, 20-22]

To obtain clearer understanding of the interaction between grapefruit juice and P-gp, it is necessary to use in vivo probes that interact with P-gp and not CYP3A4.[23] P-glycoprotein plays an important role in the absorption, excretion, and tissue distribution of digoxin,[4-6, 8, 12] but unlike many other P-gp substrates, digoxin is not metabolized by CYP3A4.[24, 25] The main purpose of this study was to determine the effects of grapefruit juice on intestinal P-gp using oral digoxin as a specific probe. A secondary objective was to determine if polymorphic MDR1 expression contributes to interindividual variability in digoxin disposition.
